                Malnutrition Education and Training Program 
                          DM&A | Nutrition Division 
                                         
           
            •  Severe protein calorie malnutrition related to 
              decreased appetite secondary to chronic COPD as 
              evidenced by po intake less than 50% of EEE for 
              greater than 1 month, 2+ pitting edema, and 12% 
              weight loss over 6 months. 
           
               
            •  Moderate protein calorie malnutrition related to 
              difficulty swallowing secondary to chemotherapy 
              as evidenced by po intake less than 75% of EEE 
              and estimated 3% weight loss over the past 2 
              weeks and mild muscle wasting and fat loss in the 
              upper extremities. 
               
              Note: recommend more specific detail NFPA / exam findings 
              should be documented in the patient chart in the clinical 
              assessment tabs.
