                                         The surgical nutrition process
                                                All admitted patients are nutritionally screened 
                                                 All nutritionally at risk patients are assessed 
                                               All high risk patients are given nutrition care plans
                                                 Monitoring of the nutrition process is done
                                                 Nutrition care plan modification / Discharge
                                                                                        
               The NUTRITION CARE PLAN is the third step in the surgical nutrition process: 
               •  Nutrition screening is the first step – all patients are screened to identify the “nutritionally 
                  at risk” 
               •  Nutrition assessment is the second step – all identified “nutritionally at risk” will be 
                  classified as mild, moderate, or high risk 
               •  The third step is the NUTRITION CARE PLAN which consists of three main components: 
                      •  Designing 
                      •  Formulating – implementation part 
                      •  Carrying out – implementation part 
               •  The fourth step is the monitoring process of the nutrition care plan implementation 
               •  The fifth step is the reassessment of the nutrition care delivery process and revision of the 
                  nutrition care plan if needed
