Vaccination Certificate of Covid-19 (Vaccine Passport)

            This is a certificate proving that you have been vaccinated against Covid-19 by the City of Shizuoka in
            accordance with the Immunization Act. There are two types of certificate, a “Vaccination Certificate
            Including Passport Details for Use Internationally and Within Japan” and a “Vaccination Certificate Not
            Including Passport Details for Use Within Japan”. Both are available as electronic and physical
            certificates.
            Eligible Persons:

            Persons who were vaccinated according to Japan’s Immunization Act  using a vaccination ticket (       
              / sesshu ken  ) from Shizuoka City , and who were residents of Shizuoka City at the point of vaccination.
            How to Apply:
            The application method varies depending on whether you want a “Digital” or “Physical” certificate.
            When applying, please also specify whether you want a certificate “For Use Internationally and Within
            Japan” or “For Use Only Within Japan”.
            Application Method 1: Application for Digital Certificate
            Download the “Vaccination Certificate of Covid-19 (Vaccine Passport)” smartphone app.
            You can apply for and receive the digital certificate within the app (instructions in
            Japanese only).
            Required:
            1. My Number Card

            *It takes time to issue a My Number Card, so please apply well in advance.
            2. Smartphone 
            *A device that can read your My Number Card (NFC Type B compatible device) 
            *iOS13.7 or later; Android OS8.0 or later
            3. Passport (only if you want the certificate for international use)

              If there is an error in your vaccination record
                  We will revise your vaccination record. Please upload a picture or scan of your “Certificate of
                  Vaccination for Covid-19” (接接接接接接/ yobo sesshu zumisho) or your “Record of Vaccination for Covid-
                       接接接接接
                  19” (       /sesshu kirokusho) with the following online form (form is in Japanese).

                  *The revision process requires some time. Please apply as soon as possible. Please wait about 10
                  days and then apply through the application again.
             Application Method 2: Application for Physical (Paper) Certificate 
             You can submit physical documents to receive a paper certificate. Please apply well in
             advance.
             *Important note for persons who intend to use the certificate within Japan.
             Both your “Certificate of Vaccination for Covid-19” (接接接接接接/ yobo sesshu zumisho) and
             your “Record of Vaccination for Covid-19” (接接接接接/sesshu kirokusho) can be used as
             proof that you have been vaccinated. If you have one of these, in principle, it is not
             necessary to apply for the “Vaccination Certificate of Covid-19 (Vaccine Passport)”.

             Submit the following documents by post to:
                                        “Covid-19 Countermeasures Division, City of Shizuoka
                                            5-1 Otemachi, Aoi-ku, Shizuoka City 420-8602”
             If everything is in order, your certificate will be issued approximately 7 to 10 days after we receive your
             documents. Please make sure there are no errors in the documents you submit.

            2. Copy of a personal identification document (My Number Card, driver’s license, residence card, etc.)
            3. Copy of “Certificate of Vaccination for Covid-19” (接接接接接接/ yobo sesshu zumisho) or “Record of

            4. Copy of passport, showing the picture of your face (only if you want the certificate for international
            use) 
            5. Return envelope (please paste a \84 stamp on it and write your address) 
            *Additional documents required in special cases: 
            When applying via proxy
             Copy of proxy’s personal identification document
              Letter of Authorization (PDF) 
            When applying via a guardian 
             Copy of your “Certificate of Registered Particulars” (接接接接接接接/ toroku jiko shomeisho) in addition to
                 the above.
            If you want the certificate to be sent to an address other than the one on your Residence Certifice (接接接/
            juminhyo)
             Document/s confirming your current address.
            If your passport shows a previous/alternate name or surname
             Document/s confirming the previous/alternate name or surname.
