   to be filled out? 
     What needs to be turned in before the 
     1st day of practice? (before August 
     11th)
      -   All forms must be turned into the high school office and the 
          athletic fee must be paid before the first day of practice.

           -   Athletic Code Booklet Sign Off Sheet
           -   WIAA Rules Exam
           -   Wisconsin DPI Parent & Student Concussion and  Head
